The enamel on the band is quite worn but otherwise

The diamonds are set in foil back settings with a heart shaped diamond at the base of the question mark

It has a locket back and tripartite engraved shoulders with an engraved shank and the four natural pearls are surrounded with gems that spell the word Dearest: diamond, emerald, amethyst, ruby, emerald, sapphire and topaz.
